Requirements for the job include:
- Clinical Care: Provide case management activities for identified patient population, including assessing and managing patients, implementing clinical practice guidelines, evaluating outcomes and modifying plans in consultation with other members of the health care team.
- Telephonic Triage and Advice Services: Provide telephonic triage and advice services for assigned population.
- Interdisciplinary Team: Participate as a member of the interdisciplinary team in the development of the patient plan of care.
- Documentation: Perform accurate documentation in medical record.
- Education: BLS (American Heart only) and CA RN required. Epic and Meditech experience highly desired.
- Travel Experience: 2 years of travel experience required.
